New patients can download the required forms, which must be completed prior to your first office visit. The forms are provided in Adobe Acrobat format.

Please complete the forms and bring them with you to the office at the time of your appointment. If you are a parent or guardian, please complete the forms for each individual in your care.

Please arrive for your first appointment at least 15 minutes prior to the scheduled time to ensure that all your information is completed correctly.

For Questions about your bill, please contact ClaimCare at 1.800.340.0455 ext. 1265. They will answer any questions and/or concerns you have regarding your bill.

At the time of your appointment you are required to bring proper identification (driver’s license) and a current copy of your insurance card, unless paying out of pocket for services.


Minors / Patient's under 18 years of age:
A parent or guardian must be present with a patient under 18 years of age on the first / initial visit. After the first / initial appointment, a Consent to Treat a Minor form can be filled out and signed for future appointments and follow-ups.

Scheduling Appointments

Our office hours to schedule an appointment are 8:00 AM to 5:00 PM, Monday through Friday (excluding traditional Holidays). When scheduling an appointment, you will need the following information:
  1. Patient Name
  2. Date-of-Birth
  3. Patient Phone Number
  4. Reason for Visit
  5. Referring Doctor (if Patient has HMO insurance plan)
  6. Insurance Plan and ID Number
NOTE: if you have an HMO insurance plan, you MUST have a written referral sent to our office at least 48 hours prior to your scheduled appointment time. You can bring the "referrral" to our office or have your Primary Care physician fax the referral to our office. The fax number for each location is above.
