Scheduling Appointments
Scheduling an appointment is an easy process. To schedule an appointment, simply call any of our locations.

Our office hours to schedule an appointment are 8:00 AM to 5:00 PM, Monday through Friday (excluding traditional Holidays). When scheduling an appointment, you will need the following information:

1. Patient Name
2. Date-of-Birth
3. Patient Phone Number
4. Reason for Visit
5. Referring Doctor (if Patient has HMO insurance plan)
6. Insurance Plan and ID Number
   

At the time of your appointment you are required to bring proper identification (driver’s license) and a current copy of your insurance card, unless paying out of pocket for services.

NOTE: if you have an HMO insurance plan, you MUST have a written referral sent to our office at least 48 hours prior to your scheduled appointment time. You can bring the "referrral" to our office or have your Primary Care physician fax the referral to our office. The fax number is 713.722.3915
